Terms of use agreement & disclosures

This agreement (“Agreement”) is entered into between Elevation Lounge, located at 5611 Norwood Ave, Jacksonville, FL 32208 (“Company”) and the individual accessing this agreement (“End User”), effective as of 12-01-2023.

1. Purpose of Agreement:
The purpose of this Agreement is to outline the terms and conditions under which the End User agrees to share their personal information with the Company and provide consent to be contacted via email or mobile phone.

2. Personal Information:
The End User agrees to provide the following personal information to the Company:

Full Name
Email Address
Mobile Phone Number

3. Use of Personal Information:
The Company agrees to use the provided personal information solely for the following purposes:

Sending promotional offers, newsletters, and marketing communications.
Providing updates about Company events, products, and services.
Conducting surveys or obtaining feedback related to Company offerings.

4. Consent to Contact:
By providing their personal information, the End User hereby consents to be contacted by the Company via email or mobile phone for the purposes outlined in section 3 of this Agreement.

5. Opt-out Option:
The End User acknowledges that they have the right to opt out of receiving communications from the Company at any time. They can exercise this right by contacting the Company directly or by following the unsubscribe instructions included in the communications.

6. Data Security:
The Company agrees to take reasonable measures to protect the personal information provided by the End User from unauthorized access, disclosure, alteration, or destruction.

7. Data Sharing:
The End User acknowledges that the Company may share their personal information with third-party service providers engaged by the Company to assist in carrying out the purposes outlined in section 3 of this Agreement. The Company will ensure that such third-party service providers adhere to data protection laws and regulations.

8. Term and Termination:
This Agreement shall remain in effect until either party terminates it by providing written notice to the other party. Upon termination, the Company shall cease all use of the End User’s personal information for the purposes outlined in section 3 of this Agreement.

9. Governing Law:
This Agreement shall be governed by and construed in accordance with the laws of the State of Florida.

IN WITNESS WHEREOF, the parties hereto have executed this Agreement as of the date first above written.
